Surveyor : Capt. K. Ali Candidate: Anonymous Result: Fail 10th Dec 2010: 0910 – 1005 All through the orals you are the Master of a UK flagged ship. Respond accordingly
1. Taken over a v/l yesterday, All initial formalities completed. Now sailing in 6 hours. How will you prepare? (I did not mention checking the voyage / passage plan) 2. Two week voyage, what will you do.ISM external audit at next port. Who will audit and what will he look for? What else will he cover under surveys? 3. Before sailing visibility drops to zero.4 NM inside a river. Will you sail? Substantiate your reasons. (I did not do a risk assessment and did not provide sufficient fodder for DPA to back me up) 4. AB reports some smoke coming from a car while sailing, Actions? ( I put out the fire under initial actions with a portable DP. Did not get to the point of raising alarm, next question was thrown in) 5. AB injured actions? (Did not mention Safety officers investigation and report) 6. CSR of this ship (As I Q. 1), CSR No.7 is certified copy. Is it acceptable? What else can be done about it? 7. C/O reports 2nd Off repeatedly coming late on watch. Suspects smell of alcohol in his breath. How will you deal with it? ( Apart from meeting ,briefing entries etc, He also wants hear how you will deal with it in a human manner) 8. ILO 178 inspection? What all does it cover? 9. NIR, Explain complete? 10. Berthing a ship in between two ships. Tide following and wind Onshore. (I said I would wait for the tide to turn around and did not consider possibility of going in , doing a short round turn, stemming the tide and then docking) 11. ISM code amendments? He wanted the exact amendments. 12. Man overboard and exact location is known. What search pattern would you use?
With this he asked me to wait a few minutes and told me I haven’t met the standards. Explained my obvious mistakes and Told me what I need to pay attention to.
Now that I have the experience of having been there; This is what I have learnt · Think about your answer, Wait and think again then answer. Do not jump and start answering straight away. · Treat every tiny incident keeping in mind that it will/could explode into a major incident(Fire etc) · The surveyor does enough to put you at ease. Your nerves should not act up. · Listen to every question completely and if not clear clarify before you answer.
